What You'll Do:
As a Clinical Case Manager within our HUD VASH program, you'll provide essential case management and supportive services to veteran participants, helping them break the cycle of homelessness and successfully transition back into the community after their release from custody.
Mobile and Field-Based Support: Provide case management services in the field, focusing on independent living skills, housing stabilization, financial management, community integration, employment assistance, benefits enrollment, and connections to vital VA partners and community providers for substance abuse, primary and mental healthcare.
Strengths-Based Approach: Utilize a strengths-based case management model to empower veterans to achieve and maintain stable employment and housing.
Collaborative Planning: Partner with clients and other VASH and VA staff to develop personalized Housing Stability Plans (HSPs) that address their short-term and long-term goals.
Ongoing Support and Monitoring: Provide continuous case management support, assessing progress on HSPs and ensuring treatment plan outcomes are met or adjusted as needed.
Empowerment and Skill-Building: Deliver supportive services focused on enhancing clients' ability to independently problem-solve, develop effective coping skills, and manage and coordinate their own care.
Evidence-Based Interventions: Utilize evidence-based practices such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention, and Housing First approaches.
What You'll Bring:
Clinical Expertise: Knowledge of and experience with advanced and evidence-based interventions, including Critical Time Intervention, Harm Reduction, Crisis Intervention, and Motivational Interviewing.
Tech Proficiency: Proficiency with computers and relevant software, including Microsoft Office Suite and database systems.
Compassionate Approach: A non-judgmental and respectful approach when working with individuals facing multiple case management and health needs.
Professionalism: Demonstrated professionalism and mature judgment.
Motivation: High level of motivation and a self-starting demeanor.
Adaptability: Flexibility, adaptability, and the ability to work effectively in a fast-paced environment with shifting priorities.
Minimum Qualifications:
Master's Degree in a Social Service or Clinical field from an accredited program (MSW, MFT, Clinical Counselor, Psychology, etc.).
Registration and accreditation (or the ability to obtain) with the California Board of Behavioral Sciences (e.g., ACSW).
One (1) year of experience in healthcare, case management, homeless services, or working with vulnerable populations.
Mandatory Requirements:
Flexible Hours: Ability and willingness to work flexible hours, which may include evenings or weekends.
Employment Eligibility: Employment eligibility verification.
CPR/First Aid: Ability to obtain CPR/First Aid certification.
COVID-19 Vaccination: Proof of full COVID-19 vaccination.
Background Checks: Successful completion of a Tuberculosis test, background screening, and drug test as conditions of hire.
Driving: Driving is an essential function of this position.
